Appointments are accepted between 8-5 Monday - Friday.
Paying for Services
Payment can be made using cash, check or charge. If you know that you want help now but are not sure if you can pay for it right now don't let that stop you from getting the help you need. Inquire about setting up a payment plan.

Using Insurance
There are pros and cons to using a managed care plan to pay for services. This is a decision you will need to consider before utilizing your benefits. If you would like to discuss your concerns about what information your managed care plan would require to process a claim or how that could impact you in the future just email me or give me a call to discuss your situation.
I do participate in several EAP and insurance panels as an in network provider. If you will be using an EAP or insurance plan to assist with your fees please contact my billing service prior to our first visit. They will take care of obtaining authorizations, verifying your benefits, and submitting claims. You can contact Pamela with Progressive Solutions by calling 678.894.4900.
The forms below are needed for all new adult clients. The HIPPA form is for your reading enjoyment. You do not need to bring in a copy of the HIPPA form to your appointment.

The street address is 2801 Buford Hwy., Ste. 503, Atlanta, GA 30329. This is about 1/2 way between N. Druid Hills and Lenox Road on the east side of the street. If you are taking MARTA it is on the #39 bus line.
